The Clinical Pharmacist plays a key role in supporting safe, effective, and evidence-based medication management within the hospital. The role requires active involvement in clinical processes, including prescription review, medication monitoring, and patient counseling. Clinical Pharmacists contribute to improving treatment accuracy, reducing medication errors, and promoting rational drug use throughout the hospital.
The Clinical Pharmacist is responsible for reviewing prescriptions to ensure correct drug selection, dosage, route of administration, and duration of therapy. This includes identifying potential drug interactions, contraindications, and adverse drug reactions while supporting physicians with medication-related recommendations.
The role involves monitoring ongoing drug therapy and participating in clinical discussions to improve treatment outcomes. The Clinical Pharmacist also plays an important role in counseling patients and caregivers on proper medication use, side effects, and adherence to prescribed therapies.
In addition, the Clinical Pharmacist ensures proper storage, handling, and documentation of medications in accordance with hospital protocols and regulatory guidelines. Supporting quality improvement initiatives, antimicrobial stewardship programs, and training of pharmacy interns or junior staff may also be part of the role.
Candidates must possess a recognized qualification in Pharmacy such as B.Pharm, M.Pharm, or Pharm.D and be registered with the State Pharmacy Council. A strong academic foundation in clinical pharmacology and therapeutics is essential for this role.
Prior experience in a hospital or clinical pharmacy setting is preferred, as it provides familiarity with prescription workflows, patient care processes, and multidisciplinary coordination. However, motivated candidates with strong clinical knowledge and willingness to learn will also be considered.
